What this site collects
No operational lead form, patient account, or medical upload is enabled. The appointment-brief tool runs in your browser and does not send its entries to the site.
Do not submit health information
Do not enter names, birth dates, record numbers, diagnoses, reports, detailed symptoms, medications, or other identifying health data into any public inquiry channel.
Before any intake opens
Any future public intake must identify the legal operator, hosting and analytics providers, contact owner, data categories, purposes, retention, deletion process, and any applicable health-data obligations before collection begins.
